MedicAlert^® Foundation membership is a fee of US$30 (stainless Steel braclet) plus P+P or US$7 ( plus P+P air, Africa only).

Annual Renewal Membership Fee US$15. Updates and replacement emblems will incure additional costs.

New Membership application form. Print the form, then fill in and have checked and signed by your Medical Practitioner, Pharmacist, or Optician.( You can fill the form in online and then print. Set printer margins to T-5mm, L-10mm, R-10mm,B-15mm. Click 'check box' to add a tick.)

Emblem Replacement form. Print out the form and fill in. This form also includes items such as chains, clasps and cleaning.(You can fill the form in online and them print. Click 'check box' to add a tick.)
